Position : Intake Coordinator – Home Care
The Intake Coordinator assists new patients with homecare needs as well as accepts new referrals from our partners. The Intake Coordinator must maintain open lines of communication with referral sources, Service Coordination, Operations, and within personnel in the Nursing/Contract departments. The Intake Coordinator will promote Americare by means of positive customer service to our partners and our members.
Qualifications :
- High School Graduate or GED
- Two years experience working working in home care or related field
- Strong organization and computer skills
- Experience with Microsoft Word, Excel, and Power Point.
- Excellent communication Skills
- Ability to work within a team environment
- Must be fully COVID-19 vaccinated
