Reporting to the Clinical Director, you will be responsible for the clinical oversight of individuals diagnosed with mental illness and co-occurring disorders. You will provide crises intervention, direct support, medical monitoring and oversight, assistance with daily living skills and other assistance needed. You will also assist in in the development of the Initial Plans, Relapse Prevention Plans and Support Plans.

In addition, you will:

· Perform assessments to evaluate a variety of mental health and behavioral conditions to children, adolescents and adults.

· Develop an individualized, prescriptive treatment plan to fit the needs of each patient using appropriate diagnostic and patient placement criteria.

· Conduct individual, family, & group counseling, utilizing methods consistent with evidence-based practice

· Coordinate patient care with primary care, other specialty providers, schools, and families

· Arrange for appropriate care within the FREE umbrella or through outside providers as prescribed in policies and procedures.

· Ensure that each patient’s rights are protected and that they are provided with appropriate interventions based on ability to participate, cultural concerns, and diagnostic impression.

· Participate in clinical care conferences, integrated care committees, risk meetings to ensure the highest quality of care.

We require:

· Master’s Degree in human services required

· Licensure as either an LMSW, LCSW, or LMHC required

o Right candidate with no licensure might be considered, but would need to sign educational agreement and would only qualify for entry level 1 on salary scale

· Minimum of 1 years’ experience working with individuals diagnosed with mental illness

· Valid NYS Driver’s License and satisfy criteria to be a driver for FREE

· Experience working with outside treatment providers and mental health professionals

· Demonstrated ability to work in a team environment with flexibility, keeping others informed and anticipating needs is essential

· Ability to be efficient, well organized, and have excellent communication skills

· Critical thinking and sound judgment skills are necessary
