STRATFORD VNA is looking for physical therapists to join our dynamic home care team. Treat adult patients one-on-one in their home and help them regain strength and mobility, modify their environment for safety and restore independence. Work for a trusted and respected home care agency, servicing the community since 1919.
- Collaborate with the interdisciplinary team
- Create individualized treatment plans
- Assist with equipment and assistive device procurement and training
- Create home exercise plans to increase strength, balance and mobility
- Provide home safety evaluations
Electronic documentation system, per visit and full time openings available. One year PT experience a must and homecare experience preferred.
