Position: Qualified Professional (QP) RN, LPN

Requirements:

• Registered nurse (RN)
• Lincensed Practical Nurse (LPN)
• Must have attended DHS Steps for Success class within last 3 years or complete required training within 6 months of hire
• Clear a criminal background study before providing services
• Willing to travel

Qualifications:

• 2 or more years of QP experience
• Excellent oral and written communication skills
• Positive attitude
• Good team player
• Hard working and self - directed
• Trustworthy

Responsibilities:

• Conduct home visits, assess needs and develop care plans in compliance with DHS guidelines
• Communicate with clients, PCAs, etc. regarding the client's care
• Train PCAs with their new clients
• Be comfortable working with a variety of people in a variety of settings
• Assist in the supervision of staff by training, completing home visits and verification phone calls

Pay:

• $125.00 per client visit (intakes and annual reviews)
• $75.00 per caregiver training
• $50.00 per client visit (supervisory visits, choice and traditional clients)
• $20.00 per service verification phone call
• $20.00 cell phone stipend every time a QP time report is submitted during the pay period

Hours per week:

• 10-30 hours per week; part-time
